Cindy was a thirty-year-old bank executive who realized that she had a drinking problem. For example, within the past six months she has felt the need to have a drink or two before going to work, two weeks ago she failed to pass a random urine alcohol test at work, five weeks ago she got arrested for a DWI, and last but not least, for roughly two months she has started to forget what she says and does when she drinks.
Like other individuals, Cindy's involvement with alcohol started out slowly and continued at this level of involvement for quite some time due to the fact that every so often she engaged in casual social drinking. As a matter of fact, for almost four years, every time she drank, she made sure to drink in moderation. Something about her drinking, however, seemed to radically change when her husband divorced her.
Cindy got extremely sad about the loss of her husband, and as a way to quit dwelling on her negative feelings she decided to start hanging around more frequently with some of her friends who love to drink and have fun. It didn't take very long, however, before her drinking increased considerably the more often she went to and drank at happy hours, private parties, family get-togethers, dinner dates, and sporting events with her buddies.
In short, she was having a ton of fun drinking just like everybody else in her group of pals without giving much thought to the alcohol short term effects and especially about the alcohol long term effects she has been going though. Yet in the back of her mind she knew about the adverse alcohol effects on the body and on the brain and that she most likely required alcohol rehab but avoided the thought as much as possible.
One morning during her annual physical, her healthcare practitioner asked her if she drank alcohol. Not wanting to lie to her healthcare practitioner, Cindy "came clean" and said that she commonly drinks more than she should. In fact, she stated that she regularly drinks in an abusive manner. Then Cindy told her healthcare practitioner about her depression. More explicitly, she articulated that shattered relationships many times triggered a disheartening cycle of events typified by increased drinking which further led to more dismal feelings that, in turn, led to even more drinking. And this is explicitly what happened when she and her husband got divorced three years ago.
Cindy's physician then told her the following: "I am not trying to make a snap analysis, but with your medical situation e may be working with two separate problems. Consequently, I think we ought to make an appointment for you to get an alcohol abuse and alcohol dependency evaluation from my partner, Dr. Johnson, who is an alcohol abuse and substance abuse specialist."
"Whether your drinking circumstance is more linked to alcohol addiction or alcohol abuse is unknown, but I think that further exploration is reasonable. Then I think we ought to schedule an appointment for you to get a psychological exam from another one of my partners, Dr. Miller, who is a counseling psychologist."
"I want to get a better handle on your depression and see how much your drinking and depression are interrelated." Cindy expressed her agreement with her doctor's treatment plan and thanked him for his assistance and concern.
